Scheduling Appointments
Appointments can be scheduled by calling our office (859-745-7890) during normal business hours.  Every effort is made to schedule a patient appointment at our earliest possible time.  We ask that patients contact our office 24 hours in advance if canceling an appointment.  We reserve the right to not schedule any future appointments for a patient that has missed (3) consecutive appointments.

New Patient Appointments
We ask that new patients arrive at least 15 minutes early to complete new patient paperwork. Or, paperwork can be found here on our website.  Please bring insurance card(s) and drivers license with you at time of appointment.  It is the responsibility of the patient to know if their insurance company requires a referral for an appointment with our office.

Charges & Co-pays for Appointments
Feet First Podiatry participates with a large number of insurance companies.  It is our policy to collect for co-pays and self pay charges at the time of the visit. Patients who do not have insurance are responsible for the full amount of office fees incurred at each visit.  There are some supplies (over the counter products) that are not covered by insurance that may be charged to the patient on the day of the appointment as well.  All other charges will be billed to the patients insurance company.  Any charges not paid by the insurance company will be the responsibility of the patient.

Foot Care Generally Excluded from Insurance Coverage

The following routine foot care services are generally EXCLUDED from coverage under both Part A and Part B Medicare:
• Cutting or removal of corns and callouses
• Trimming, cutting, clipping, or debriding of nails
• Cleaning and soaking of the feet
• The use of skin creams to maintain skin tone

Conditions That Allow for Insurance Coverage of Foot Care
However, routine foot care IS generally covered by Medicare and other insurance companies when a patient is diagnosed with the following conditions:
• Diabetes Mellitus
• Arteriosclerosis
• Peripheral Vascular Disease - Poor Circulation
• Chronic thrombophlebitis
• Peripheral Neuropathies

Ordering of Diabetic Shoes and Orthotics

When ordering diabetic shoes, orthotics or braces, ½ of the total fee must be paid before ordering. The balance must be paid at time of pick up.  Generally, it will take 4-6 three weeks for the order to be delivered to our office.

There will be a $25.00 Fee for the following paperwork:
Paperwork associated with Workers Compensation
Short/Long-term Disability paperwork
FMLA Paperwork
Medical Necessity Forms

Returned Check Fee

There will be a $50.00 charge for all returned checks.
